Superior Vena Cava Syndrome

Obstruction of the superior vena cava (SVC) may be caused by neoplastic invasion of the venous wall associated with intravascular thrombosis or, more simply, by extrinsic pressure of a tumor mass against the relatively fixed thin-walled superior vena cava (SVC).

Differentials for SVC obstruction

Malignant

1. Malignant mediastinal tumors

2. Bronchogenic 70-85 % small-cell carcinoma

3. Non- Hodgkin lymphoma

Nonmalignant conditions

1. mediastinal fibrosis

2. vascular diseases such as aortic aneurysm,

3. tuberculosis, syphilis, and actinomycosis;

4. Benign mediastinal tumors such as teratoma,cystic hygroma, thymoma, and dermoid cyst

Plethoric or cyanotic facies. The eyes may show exophtalmos, conjuctival injection, venous dilatation in the fundi, JVP raised, thyroid may be enlarged, supraclavicular lymphadenopathy, Pemberton's signs may be positive.

Tracheal compression, may show stridor, and respiratory distress.

Hoarseness of voice

May associated with Horners syndrome,

May associated with paralysis of phrenic nerve, paralysed diaphargm.
